At Home

You may be surprised how many seniors, even those with some significant physical and cognitive challenges, can remain at home with the right home care support.
Many families and even some healthcare professionals think that a particular senior should go into some type of support facility but many times home care can help the senior stay at home with no to minimal risk.

Home is where most seniors really want to be. It is good for them mentally and physically.

Toronto Senior Care can provide can virtually all forms of non-medical home care support from a few hours a day to 24 hours a day.

We treat each senior with respect and great care. Each seniors needs are unique, as unique as themselves as people.

When you contact Toronto Senior Care we can certainly provide you with much of the information you require right on the phone. We can also mail or email you more information to assist you.

In your call with us we ask some basic questions to understand your specific situation and needs so we can better answer your questions.

If you wish even while on the initial phone call we can perform a preliminary telephone assessment, gathering some key information to assist in the process of determining specific needs and service requirements.

We can also setup a home visit to meet you and perform a no-charge, no-obligation Home Care Assessment.

From there we develop a documented Home Care Plan for the senior that details all the necessary information that allows a caregiver to understand the seniors challenges and needs each day. You can review this plan as it can be mailed or emailed to you for approval.

If you decide to proceed with our services we will ask you to sign a short Toronto Senior Care Service Agreement which simply details the terms and conditions we provide service to the senior during the time we are doing it.

For those seniors who are receiving home care services from the CCAC we can work with their schedule to supplement care and make it more affordable for the senior.

In A Retirement Home Well Senior Car

Nursing homes are sometimes called Long Term Care facilities.

Long Term Care Facilities are designed to provide care for seniors and others who have significant care needs.

Seniors who live in Long Term Care Facilities may be physically disabled, suffering from dementia or both. They may be bedridden and/or be unable to feed themselves or take care of their own personal hygiene. They may be prone to wandering, and as a result be on a “locked floor”. They may even be a risk to themselves or others.

In Ontario, Long Term Care Facilities must be licensed or approved by the Ministry of Health and Long-Term Care and are are governed by the Long-Term Care Homes Act, 2007 (LTCHA). Wait lists for specific facilities can be long, as much as two years, and there is no guarantee that the long term care facility most convenient to a Senior and their family is the one that will be offered.

In Long Term Care Facilities nutrition and hygiene are key. However, staffing ratios of up to 15 clients to per caregiver during the day and 20 at night are not uncommon. Assisting a frail senior who has mobility issues,dementia or Parkinson’s Disease (to name but a few of the possibilities), can be extremely time consuming. Facility staff often find that they are stretched to the limit trying to ensure that the seniors they are caring for receive the nutrition they need and that they are properly hydrated.

Similarly, ensuring that clients, many of whom are incontinent, are kept clean on a day to day basis as well as bathed (Facilities must abide by Ontario Government guidelines mandating clients are bathed two times per week) can also be very challenging.

Clients who are bedridden must be repositioned on a regular schedule or are risk of developing “bed sores”, which on their own, can be life threatening.

Many families have used Toronto Senior Care’s staff and services to supplement with the LTC facility provides to ensure their loved one is receiving excellent and respectful care.

In Hospital

We are often called on to provide supplemental care for seniors who are in the hospital.

The two most common scenarios we come across are; seniors who have found themselves in the hospital as the result of a sudden crisis, and seniors who are in the hospital as the result of planned surgery (a planned hospital stay). In both cases, having supplemental care while in the hospital, can make a real difference.

Crisis:
Often seniors who have found themselves in the hospital as the result of a crisis are completely unprepared for the stress, anxiety and confusion that they initially experience.

Communication can be challenging. Seniors and their families often find it difficult to understand what is going on, who is supposed to be doing what, and when. It is at this time of crisis that they realize that the medical community is a world unto itself; with its own way of doing things, its own schedule and its own language.

Toronto Senior Care’s caregivers can assist hospitalized clients with timely visits to the bathroom, showering, dressing, walking and eating. They can provide companionship and mental stimulation. They can also communicate concerns to the hospital staff and our own Registered Nurses, to help ensure action is taken when required.
Knowing a caregiver is on site can also provide the reassurance families need, so that they themselves, can take a break.

It is important for families to understand that there are people at the hospital who can help. A Social Worker can help you understand what is going on, what your options are and what to expect. Your Social Worker can also, when the time comes, help you access resources in preparation for discharge.
Toronto Senior Care can help both during the hospital stay as well as with the discharge and return home.

Palliative Care

Toronto Senior Care can provide around-the-clock, 24 hour care for palliative clients. We can also provide short term relief for family caregivers.
Palliative clients and their families make the decision to stay at home with care for a variety of reasons. For many, the familiar surroundings of home means they are more relaxed and less stressed, and are more able to participate in daily activities.

For family it means little or no travel time is involved to spend time together. Visits can be more frequent, more “normal”, less clinical and more pleasant.
Our staff are familiar with the specialized needs of palliative clients and the resources that are available in the community. We help our clients access all of the resources they need.

It is often a relief to clients and their families to learn that they are able to return home, even when they require various types of devices, therapies and medications.

Acquisition of equipment for short or long term use can be arranged. Hospital beds can be rented or purchased. Grab bars can be installed in showers etc. and a variety of other aids can be put in place to enhance mobility and safety.
